“ChatGPT Prescribes a Painkiller” If You Don't Say Which One, You're Dead

BBE PoV - KEY TRENDS IN PHARMA · JUNE 2026

“Being chosen now has two completely different mechanics running at once.”

THE OBSERVATION THAT REDEFINED MARKETING

Gary Vaynerchuk made a simple observation that quietly redefined marketing: "Alexa, order me a pizza." If you didn't say Domino's, you were dead. The agent ordered for you — and it ordered for whoever had bothered to be the answer.

That line is no longer about pizza.

Today (yes today…not in a near future as you might start thinking) the same sentence runs through hospital systems, prescribing software, and the AI copilots are starting to query before they query a colleague. ChatGPT prescribes a painkiller. If you don't say which one, you're dead.

Painkiller just to say a category… IS FOR EVERYTHING!

That infrastructure is being built right now, while most pharma marketing teams are still optimizing the funnel that made them successful for the last twenty years….

“It is time to start thinking about your new invisible customer.”

The funnel that built the industry won't be the one that runs it.

Pharma marketing has lived inside a simple, human logic for decades: Awareness → Interest → Decision → Action. The physician sees the brand, builds trust, weighs clinical evidence and relationship, and writes the prescription. Irrational factors — familiarity, loyalty, a Reo conversation, a congress hallway — have always had a seat at that table. It's a funnel built for a brain. A second funnel now runs in parallel, built for a machine.

Why this isn't 2030 thinking

It is tempting to file this under “interesting, future, not urgent.” The data says otherwise.

None of these examples come from pharma, but the lesson is identical — arriving from a different industry first, the way SEO arrived through Google before it ever touched anyone's category, and the way SEM, then GEO, then AEO each rewrote the rules of visibility roughly once a decade.

Pharma is simply a few steps behind this pattern, not exempt from it. Merck is deploying agentic AI across its entire value chain in a partnership worth up to $1 billion. Novo Nordisk has integrated AI agents from drug discovery through commercial operations, with workforce AI fluency treated as core infrastructure, not training. The boundary between “technology company” and “pharmaceutical company” is dissolving from the inside.

The brief nobody has written yet.

Whatever that answer is, those criteria are the new marketing brief — ahead of the campaign message, ahead of the brand story. The structured, machine-readable, queryable data is what determines whether a product is even in the conversation before a human ever sees it.

Most companies don't have that data in a form any agent could read. That gap belongs to marketing, not IT — because marketing is the function that has always been responsible for being chosen, and being chosen now has two completely different mechanics running at once.

“Here is the uncomfortable question every brand/marketing team should be asking: what criteria would a HCP program into their AI agent to select a treatment for a pathology?”

Where this leaves marketing teams.

We've run this question with marketing teams and the pattern is consistent: the attitude is already there. Curiosity is high, openness is high, nobody is resisting AI. What's missing is depth — a real grasp of what an agentic funnel actually requires, and the skills to build for it before the infrastructure decisions get made by someone else.

That gap closes in one of two ways.

PATH 01

Build it deliberately, now

A team builds the vision and the capability deliberately, now, while the window is still open.

PATH 02

The market closes it for you

Or the market closes it for them, the way it always does, by simply moving on to whoever was ready.

The teams that treat this as 2027's problem will spend 2027 trying to catch competitors who started in 2026.

LET’S TALK!

Bluebarna Ecosystems works with pharma and healthcare organizations to build the capability that the agentic shift requires. If this resonates with where your team is right now, let's talk.
